DaVita Inc. provides kidney dialysis services for patients suffering from chronic kidney failure or end stage renal disease (ESRD). As of December 31, 2020, it provided dialysis and administrative services in the United States through a network of 2,816 outpatient dialysis centers serving approximately 204,200 patients; and operated 321 outpatient dialysis centers located in 10 countries outside of the United States serving approximately 36,700 patients.As a Renal Social Worker, I was responsible for ensuring that our patients had the resources they needed to make dialysis treatment and transition to transplantation easier: ✓Advocated for patients and ensured they had the resources needed to make treatment easier. ✓Conducted initial psychosocial assessment and interview with each client. ✓Served as a key point of contact for 100 clients seeking emotional support, transportation, and financial resources. ✓Collaborated with a multidisciplinary team of doctors, nurses, patient care technicians, administrators, and dietitians to design long and short-term treatment plans.
