I am a Social Worker currently working in a Discharge to Assess Team (D2A) in my role I mainly work with service users who are in the hospital setting, care homes and in their homes. I was previously working in the Social Care Assessment Team and in a different Discharge to Assess team (D2A). I have experience working with individuals and families with ongoing care and support needs. I have worked with doctors, nurses and physiotherapists to meet service users care and support needs, in the hospitals, care homes and in the community.
