•Evaluation and continuous quality improvement; utilizing the Model for Improvement, and other quality improvement methods to identify and resolve gaps in system policies and clinical workflows in the primary care setting.•Using community-based program planning and implementation to enhance the quality and accessibility of services for patients experiencing health inequity by promoting access to care among federally qualified health centers and the medical community. •Facilitate health education and promotion for patients and clinical staff on clinical guidelines and evidence-based interventions.•Build and maintain a coalition of stakeholder partnerships with hospitals, charity care networks, and local area health education centers to bring heightened awareness of cancer prevention and early detection •Quality improvement coach for a portfolio of Federally Qualified Health Centers (FQHC’s) in North Florida delivering on the most effective and appropriate evidence-based strategies to achieve improvements in cancer screening outcomes. •Provide technical assistance to community health and primary care systems on the integration of community health workers and other prevention and screening navigators into clinical practice. •Mobilize community stakeholders to strategically address the availability of diagnostic screening and cancer treatment for the uninsured/underinsured in North Florida. •Manage grant funded quality improvement projects and community empowerment grants to improve health equity, access, and navigation to screening resources in North Florida and Southwest Georgia. In addition, provide programmatic support and technical assistance to grantees. •Assess health system capacity and readiness while working alongside health systems leadership to develop strategy to achieve each systems cancer control priorities. •Collaborate with diverse populations and health systems on prevention, early detection, screening, and access to care initiatives.
