Implementation of a home- based recovery program for patients discharged from the acute hospital setting with cardiac and pulmonary diagnoses. In collaberation with a multi-disciplinary team including RT, a pulmonologist and cardiac consultant, this program is designed to decrease hospital re-admissions of patients with diagnoses such as COPD, CHF and Pneumonia and other related illnesses.
