Partner and collaborate with all stakeholders and cross-functional departments, including Senior Care, Complex Care, Transitions of Care, Specialty Services, Behavioral Health, Hospital Partners, Quality, Nursing, Health Education and Wellness and Health Services to optimize delivery of care services to impact patient experience, quality, utilization and health outcomes while maximizing access and cost-effectiveness.
