Registered Nurse/Case Manager
-Staff Nurse on a multidisciplinary team serving 350+medically fragile children with complex illness-Triage questions/concerns from families via phone & electronic messages-Clinic check in and medical procedures: GT changes, urinary catheterization, injections, etc.-Prep Provider orders to Nursing companies, DMEs, and therapy providers-Prior authorizations for medications/equipment, and appeals of these when needed-Assisted in development of patient plan of care prototype; currently being trialed-Obtained data for research project on patients with complex medication lists at hospital discharge
