I combine my past knowledge and experience in the health care sector with my new lived experienceMy lens is using lived experience with an elderly mother in law, keeping her healthy and safe in her retirement home, and also the lived experience of supporting my family struggle as their son was diagnosed with lung cancer and died within 5 1/2 months.

-provide targeted to in-depth clinical assessments of patients referred for a Geriatric Consultation- Working member with the CNA on the development of Choosing Wisely Canada statements for gerontological nursing-co-lead for the development and knowledge translation activities for the Best Practices in Senior Focused Hospital as part of the Seniors Strategylead for the Senior Friendly Hospital ACTION project at Mackenzie Health focusing on reduction of post op functional incontinence on a rehab unit. -co-lead for the implementation of the 2 RNAO Best Practice Guidelines related to delirium, depression and dementia- provide leadership in the planning and delivery of an annual Seniors Health Clinic Day for front line clinical staff- actively participated in the Transitions of Care work group to implement seamless transition in care from community to hospital to long term care-co-facilitate Gentle Persuasive Approaches in Dementia Care as a certified coach for staff-facilitate knowledge translation activities for new hires to the organization-ethics facilitator.-planning committee member for the new Mackenzie Vaughn Health Hospital - using a Seniors Lens.- obtained by GNC(C) designation from the Canadian Nurses Association

- re-launched the role of the GEM nurse in the Emergency Department using the Framework developed by the Regional Geriatric Program- provided comprehensive, specialized and advanced nursing assessments and care planning for high risk Seniors in the Emergency Department- lead the development and facilitated knowledge translation sessions of Senior specific educational modules for use within the Emergency Department- co-lead for the organizational project management of the 2 RNAO Best Practice Guidelines related to Delirium, Depression and Dementia with the Best Practice Spotlight Organization Candidacy period. -provided evidence informed resources during the Functional Planning Process for Geriatric Assessment Rooms- lead the development and implementation of Discharge Phone Calls to Seniors in support of the Corporate Widely Important Goals.- member of Central Local Integrated Health Network - developing orientation module for new GEM2012 - maintained by ENC(C) certification

In addition to my role as CNS Seniors Health at Mackenzie Health, I am currently also the Lead for the Western York Region Health Link. Working very closely with Home and Community Care and my other 20 + community partners we are providing coordinated health care and care plans to complex residents of the South West Sub region. We have renamed to sub region to Western York Region - to align with the sub regions of the Central LHIN. I am looking forward to the establishment of the Sub Region Tables and the alignment of the Health Link philosophy into this nex step in regional health care.I have recently been named to the Western York Region Subregion planning table.

- lead knowledge translation activities prior to and during move to new Emergency department- lead the review of a comprehensive 20 competency based orientation for new hires- lead the project for certification of the glucose meter for agency nurses working in the emergency department- lead the development and provided ongoing support for the Rapid Admissions Team

- management of both human and financial resources for large community hospital- lead the development of the New Grad Initiative within the Emergency Department with the employment of 13 new staff- initiated a unit council within the Emergency Department- participated in a third party external review of the Emergency Medicine Program.

- lead the development of a competency based orientation program for newly hired nurses, refined and facilitated return to work and return from leave self directed review and update programs.- provided subject matter expertise for the development and implementations of the Critical Care - Emergency Nursing Course at Seneca College. Taught in class modules related to Triage and Paediatrics. Mentored staff during clinical-developed medical directives for the Emergency Department in collaboration with Clinical Chief for both adult and paediatric patients- seconded to cover Neonatal Intensive Care Unit, Paediatric Unit, Medical Ambulatory Care, Fracture Clinic, Diabetes Education Unit and Endoscopy Suites for up to 12 months. - member of various working groups for corporate policy development both within the Emergency Department and Professional Practice

- lead the development of the role in an out patient setting- provided comprehensive, and specialized nursing assessment to high risk patients- lead the development of patient discharge phone calls to high risk patients focusing on quality of their care and awareness of discharge instructions.- co-lead for the development of a Deep Vein Thrombosis Protocol which was an admission avoidance strategy.- collaborated with York Region Public Health in the development of an in home falls preventions program
