Provide intensive care management for individuals to ensure a successful transition into community-based living and treatment from state hospitals, acute treatment centers, crisis services facilities, withdrawal management facilities, criminal justice settings, or other higher levels of care. We care for individuals, families, and children who receive health care under Child Health Plan Plus (CHP+) and Health First Colorado (Colorado's Medicaid Program).As a Care Manager, I am responsible for completing the appropriate level care management activities for Colorado Access members. Care management activites include, completing assessments, determining eligibility, monitoring provider services, coordinating services, developing care plans,delivering care management interventions, appropriate follow-up activites, and completing all documentation in the expected time frame.Apply the appropriate criteria, guidelines, regulations specific to the level of care and services required to meet the members/family goals while staying within the organizational/contractual requirements.Care management functions are performed for members in a variety of settings; telephonic, in-person, or in the community setting.My position is in the Single Entry Point (SEP) division, traveling to members in Adams, Arapahoe, Douglas and Elbert counties to access their needs for Long-Term Care (LTC) services, either to stay in their home and community, or to go into a nursing home facility for more intensive care.

--The research project developed a proposal to further health care outcomes on the state level. By using existing empirical data and primary research, the project analyzed global and domestic health care delivery models to determine best-use efficiencies in designing a new state-based health care delivery model. --The model may increase the likelihood of achieving the "Triple Aim", by controlling health care costs, allowing for better access and population health, and improving the client and provider health care experience. --Institutional Review Board (IRB) approval was obtained to survey members of the 2014 Colorado General Assembly on issues concerning state and national reform efforts. Nearly 80% response rate was achieved from the 100 legislators. --The unique policy position was submitted to the legislators after completion with positive feedback for consideration in future legislative sessions.

--Gather information and provide analysis on a wide range of federal, state, and local legislative issues related to health care and health care outcomes. --The most significant achievement was the preparation of a report entitled "Colorado Super-Utilizer Activities: A Primer". Interviews were conducted with various stakeholders including Regional Care Collaborative Organizations (RCCO), to describe diverse pilot programs to offer more access, improve delivery systems, and reduce health care costs for Colorado Medicaid clients. These clients have complex needs that have not been met despite unnecessarily high utilization of the health care system. --The purpose of this examination was to provide an overview of several current super-utilizer activities underway within Colorado in order to inform the discussion moving forward and build upon initiatives already working, as part of the National Governor’s Association multi-state policy academy. --Research was also conducted to ascertain the involvement of other Governors' activities in promoting the Affordable Care Act, state insurance exchanges, and the Medicaid expansion. --Policy recommendations were provided regarding Freestanding Emergency Rooms, the Medical Clean Claims Transparency and Uniformity Act, and the Prescription Drug Monitoring program.

--Advise the owner of this specialty physical therapy practice on strategies to streamline operational processes that would allow for greater financial stability, growth of practice, providing superior customer service, and lowering costs. --Create profit and loss statements for the practice along with balance sheets, reported daily cash logs, sent patient records with diagnostic codes to medical billing company and conducted scheduling with patients.
