At a high level, the dashboard provides capabilities to track targeting effectiveness, ROI trends and KPI monitoring for each program. • Apply analytic insights to target the right members for the right services. Evaluate the entire member population and prioritize candidates for case management to identify members with the highest expected need for services. Direct clients to appropriate actions including telephonic and field-based services. Help win and retain member enrollment and improve population health.

• Design and refine customized value-based targeting rosters for 12 provider groups. Monitor the performance of member stratification system. Examine frequency results to look for shift, drift and anything out of the ordinary. Lead risk stratification on multi-comorbidity using health records for Patient Center Medical Home project. Build simplified risk stratification algorithm and provide a feasible option for clinic setting. Manage large scale healthcare database including 29 states Medicaid claims data of over 54 million individuals, CMF, ARF, NPI database, Medicare Part D data, etc. Run SQL data queries and merge and create data sets in alignment with study needs.
