Discharge Manager
CurrentI have many years of experience of dealing with complex discharges, crisis prevention and re-admission of patients to the hospital following discharge. I work as part of an integrated discharge team in a very busy hospital linking in with the bed managers and the operational managers and directors to ensure bed capacity is constant and reduce delays. I also manage a team of ward based Discharge Officers who are trained to deal with complex patients needs and identifying and implementing discharge plans. I love my work as no two days are the same and there is always a challenge which I thrive in. I work as part of the multidisciplinary team liaising and working together with medical staff, therapist, ambulance transport, and other services within the hospital and in the community. I have vast knowledge of every aspect of discharge planning and am well known for my trouble shooting skills.I often train new staff about discharge planning as part of their corporate training and am always interested in expanding my knowledge too. I adapt to change easily and always look for new ideas to work smarter and more effectively.
