The strategy involved enhancing communication between providers and the patient, providing alternative avenues of care for the patient besides the ER, as well as leveraging technologies and resources from UPenn to create a seamless transition of care for ESRD patients. The strategy is projected to reduce patient costs and hospital readmission rates >10%, improve the quality of transitional care for high risk patients, and increase revenue for all providers involved in the ACO. The final product is slated be implemented at UPenn hospitals and marketed as a model for DaVita to use nationally.
