Being a Certified Community Health Worker is a fairly new here in Michigan and for our Physician organization and I am the CCHW. I am excited to bring my greater than 30 years of Health Care experience with me. When care managers have patients that need assistance with SDOH needs, they are referred to me for assistance in finding and using resources.

Supervisory role of 6 FTE's rehab assistants in a residential Closed-Head Injury Rehabilitation facility. Active role in carrying out Medical Doctors and all other provider therapies orders. One on one care provided to patient. Medication reconciliation, ordering and dispensing, Behavioral management strategies utilized.

Provide complete care of the geriatric patient. Assist in all therapy models and take and record vital signs and assist with medication passing.
