Responsibilities:• Report to program manager.• Provide support to participants in coordinating services through the Medicaid Waiver Program. • Support the mission of my agency by providing person and family-centered support. • Prioritize and have a sense of urgency for enrolling participants in various Medicaid programs throughout the state of Maryland. • Comply with specific contract requirements, integrating technology to maintain database integrity, report writing and maintenance of documentation. • Provide weekly and monthly updates, written reports, and deliverables updates to clients and supervisors. • Research various housing programs/ facilities willing to accommodate those participants looking to become enrolled in the community options waiver program. • Conduct person-centered meetings with participants and their families to review Plan of Care Assessments and Plan of Services for enrollment in Community First Choice Program.• Service 9 counties in Maryland to provide Supports Planning services. • Review client’s profiles, update case notes, complete monthly monitoring, 3 month visits, develop Medicaid Plans, and upload participants supporting documentation into the LTSS Database. • Communicate with participants of various backgrounds, co-workers, and government officials in regards to participant’s eligibility status and updates.

• Monitor and answer phones calls and transfers• Track packages• Provide excellent customer service to residents at apartment complexes in the Washington D.C, Maryland, and Virginia area.

Washington D.C. Metro Area

• Supervise 3-5 years old in summer youth activities. • Supervising 3-5 year olds in summer camp activities with the Little Explorers Program. • Provided various learning schedules, field trips to museums, pools, parks, and providing safety for the explorers and communicating with parents daily.

• Provide excellent customer service to those entering and leaving the store.• Stock merchandise in shoe department• Prepare for sale events• Answer and transfer calls• Open and Close registers

Washington D.C. Metro Area

• Office duties: Answering phones, filing, providing customer service to agents while working in the health department area. • Completing all tasks given to me by Mrs. Ursula Jefferson-Flowers intern supervisor.• Scan documents, send emails, and maintain confidentiality of Special Agents health records at Washington field office. • Attend Summer Intern Meetings.
