Responsible for 39 Skilled Nursing Facilities in Western and Downriver Wayne County, Managed team of five certified Long Term Care Ombudsman volunteers and interns. Interacted with long-term care facilities in the service area on a regular basis for complaint and non-complaint related visits. Educated the public regarding all aspects of the long-term care system. Duties also included advocacy, community education, assisting individuals in accessing long-term care services and assisting in development of resident/family councils.

Managed a team of hospital-based Community Health Workers with the goal of empowering patients being discharged from hospitals to take actions aimed at reducing avoidable re-hospitalizations. Effectively focused the team on achieving individual and program performance goals. Continually trained the team to effectively enroll patients, implement the agency’s care transitions model, properly document cases and coordinate resource referrals to proper agency staff. Developed and refined processes to maximize performance. Worked with agency’s fiscal and information technology staff to insure data integrity and proper programmatic billing. Served as the program’s hospital and community liaison; Collaborated with a variety of community-based partners in the service network.

The Care Transitions Improvement Coalition (CTIC) of Southeast Michigan was organized for the purpose of reducing the 30-day hospital all cause readmission rate by 20% over five years by supporting interventions implemented under the Affordable Care Act’s Section 3026 funding, thereby improving patient and family satisfaction with the health care experience. Through shared experiences, coordinated processes and communication of evidenced based interventions, the CTIC seeks to measure and improve care transitions. The Coalition voted to disband when CMS ended the funding for Care Transitions. Our program was successful in meeting our goal.

Responsibilities were varied and included management of guardianship/conservatorship services, program management, marketing and business development for an array of programs and services designed to meet the needs of disabled individuals and senior citizens. Senior Care Network – Collaborate with discharge planners, attorneys, service providers, social workers, case managers and individuals to increase referrals, improve community options. Provide executive case management services. Responsible for significantly enhancing connections and increasing business opportunities with non-profit agencies and local businesses. Probate Court Casework - Negotiated/Awarded/Monitored major contracts with two Michigan hospital systems to provide guardianship/conservatorship services. Provided training to case management teams state wide. Petitioned in Probate Court for at-risk clients. Successfully developed a system to increase program capacity, streamline processes, insure timely service, expedite referrals, improve processes and operations. Established relationships with Probate staff and Judges. Helping Our Parents – Assisted and advised staff members in the development, marketing and evaluation of this service that was introduced to the community in February of 2011. Contracted with providers and clients to establish appropriate care plans.Homestead Home Health Care Services – Collaborated with non-profit agencies, foundations, hospitals, service providers and individuals to increase customer base for private duty home care services and develop relationships with organizations to increase community awareness and networking opportunities.
