Senior Operations Manager, Patient Navigation Center
CurrentServe as the lead and subject matter expert for developing and implementing an organization-wide approach to screening and addressing health-related social needs across our clinical, care coordination, and community engagement and outreach teams for our 100,000+ Medical Access Program members. Scope of responsibility includes transportation, food insecurity, language access, housing support, durable medical equipment, and telemedicine. Foster sustainable relationships with internal and community partners and serve as a strategic thinking partner within the Navigation Center team.● Chair of agency’s first Social Determinants of Health/Health-Related Social Needs Workgroup● Develop a portfolio of social determinants of health services and create partnerships with community organizations who will provide services● Lead our organization’s launch of FindHelp, social care resource network, including training for all community-facing staff● Serve as a subject matter expert on the Austin/Travis County Food Plan development committee for food access and consumption● Support creation, expansion and sustainability efforts for a patient experience team● Develop process workflows, evaluate effectiveness, and identify opportunities for improvement of the Navigation Center operations and services including Medical Access Program eligibility, referral coordination, social resource navigation, and nurse navigation● Identify opportunities for coordination across teams within the Navigation Center and effectively deploy change management techniques to improve program outcomes
