Critical assessment of new admissions and long-term care resident’s charts to obtain the highest possible reimbursement for the level of care each patient receives, inputting the data into the computer and transmitting for Medicare and Medicaid billing. Communication with IDT including Rehab staff for setting ARD dates, scheduling assessments and follow up to ensure assessments are completed in a timely and accurate manner. Maximizing each resident to achieve the best possible reimbursement and case mix score.

Responsible for the care of 75 dementia residents; including staffing and counseling for caregivers and nurses. Daily review of incident reports, review of resident’s plan of care and care conferences. Assessed potential new residents. Over saw nursing skin rounds, weight monitoring and ensuring proper charting. Learning the WAC and RCW. Able to teach mental health and specialized dementia classes to care givers. Close communication with Executive Director and Regional Supervisor.

Review of long term and transitional care patient’s medical records both upon admission and on a quarterly basis, according to Medicare/Medicaid guidelines in a 129 bed skilled nursing facility, Documentation of obtained information into the computer. Communication with other team members in determining the level of care each patient receives. Use of Care Tracker, implementation of a new computer system. Ensuring proper documentation papers are available to the staff to records patient’s level of care needs. Support resident care managers and assisted DNS with admissions and discharges.

Perform management of the nursing staff for 54 residents. Responsible for the care plan updates, quarterly updates, incident reports and state notification when applicable, and admissions of new patients, also rounds with physicians,
