Clinical Assessor- Responsible for coordinating and completing the initial assessment of all clients, developing an initial treatment plan, and triage clients to appropriate level of care.Intensive In-Home Team Lead - Responsible for coordinating the initial assessment and developing the youth’s Person Centered Plan (PCP)- Monitor and manage the presenting psychiatric symptoms of the identified client through crisis management, intensive case management, individual and/or family therapy, skills training, linkage to needed community services and resources, and other rehabilitative supports to prevent the need for an out-of-home, more restrictive services - Provide “first responder” crisis response on a 24/7/365 basis to recipients to diffuse crises, evaluate nature of crises, and intervene to reduce the likelihood of a recurrence- Approve timesheets and billing, ensuring accuracy and timeliness, coordinate and conduct peer review,ensure incident reports are prepared and submitted on time, and provide supervision to team membersOutpatient Therapist - Provided necessary therapeutic interventions to consumers and families based upon a jointly determined treatment plan- Responsible for conducting ongoing assessment of treatment plans, referrals to community agencies, consumer advocacy, and timely and accurate documentation of services rendered

• Responsible for the psychological and social well being of patients and patient families, documentation of provided services, referral to community resources, , and patient advocacy • Coordinate with the hospice team to develop and implement appropriate client care plansresulting in quality, comfortable, and dignified end of life care • Provided necessary counseling services and family meeting facilitation to address issues surrounding a patient’s hospice status• Initiated follow up bereavement services to patient families to provide ongoing support and to facilitate pro active grief work
