Led a process improvement project which expanded the discharge pharmacy program to the surgical floor. This program involved delivering medications to patient’s room, completing comprehensive medication counseling, and coordinating post-discharge plans including transferring refills to the patient’s preferred pharmacy. Performed continuous chart review to better prepare for future discharges, including evaluating pharmacy needs and proactively troubleshooting possible issues related to medication cost and availability. On the day of discharge, assessed patient discharge times and prioritized medication filling in the outpatient pharmacy appropriately. Lastly, tracked interventions made, including missing medications, incorrect medications, incorrect doses, among others.
