  Responsibilities included:- Conduct intake and initial assessment to determine scope of the individual's need.- Advocate for those with disabilities in their pursuit of independent living.- Make referrals to community resources beyond the scope of the disAbility Resource Center.- Assist those with disabilities in creating strategies for successful integration into the local community.- Connect those with disabilities with government (local/state/federal) resources to support their physical and mental health, living conditions, employment, and educational needs.- Provide peer mentorship counseling, to include assisting those with disabilities to learn social skills and reach individual goals.- Assist those with disabilities in their transition from nursing homes or other institutional settings to living independently in their own homes.- Create lesson plans and lead/facilitate group sessions for teaching independent living skills to those with disabilities.- Assist those with disabilities in the completion of forms for government (local/state/federal) programs (e.g. SNAP, Medicaid, educational 504 and IEP, disability benefits, veterans benefits).- Generate reports for Virginia Center for Independent Living (VACIL).
