Bilingual Case Manager II: As a case manager for NYAP’s La Jornada (The Journey) Program I focus on making a meaningful difference in the lives of minors who have fled their country of origin and arrived in the U.S. without a parent or legal guardian. I work closely with the United States ORR (office of refugee resettlement) as well as with the minors,foster families,sponsor families,and community partners to provide advocacy and support towards targeted goals.

Works directly with parents to equip them as their child’s first and most important teacher. Providesresources, referrals and information to families of young children to ensure their effectiveness as parentsand their ability to prepare their children for learning in school. Educates parents about child developmentand appropriate learning situations for young children. Fosters self determination by helping parents learn the skills they need to care for their family’s needs. Assists families in attending to the needs of children related to health care, social/emotional development, language development, physical development, family living, and cognitive growth.Provides primary service to children enrolled at the Conder Elementary, a designated at-risk, Title One population. Works closely with Conder classroom teachers to observe andinteract with students, allowing for powerful, personalized communication with their parents.Develops strategies to engage families in promoting school readiness skills. Works with special projects and populations as assigned. Some examples are teen parents, Birth To Three children, second language learners, and under-educated families. Ensures high standard of ethics by providing for family confidentiality.

I provide culturally appropriate contraceptive health education, outreach and advocating for Black and Hispanic adolescent youth ages 15-21 in Lower Richland, Monticello and Waverly areas along with the local universities. I also act as a cultural liaison to the clinical team, to aid in cross-cultural understanding and enhance quality of service.Working with Fact Forward through their Tier1 Project IMPACT grant with the U.S. Department of Health and Human Services (HHS) to support the evidence-based Teen Pregnancy Prevention (TPP) program. The TPP program is a national, competitive program that provides funding to replicate and scale evidence-based programs and develops and evaluates new and innovative approaches to prevent teen pregnancy, improve adolescent sexual health outcomes and promote positive youth development in communities across the United States.
