As a Re-Engagement Specialist at Sun River Health, I conduct outreach and coordinate patient care within an interdisciplinary team, ensuring compliance with all agency policies and procedures. I have successfully located and connected patients who need additional support and services to retain them in care, using my bilingualism and communication skills.I am also pursuing a Bachelor of Arts in Social Work at CUNY Herbert H. Lehman College, after completing an Associate's degree in the same field at CUNY Bronx Community College. I am passionate about helping people and making a positive impact in their lives. I have gained valuable experience in the hospital and health care industry, working as a Patient Care Coordinator at Friends & Family Homecare and a Patient Care Advocate at Brightpoint Health.
