For over 10 years I’ve helped strengthen the health and well-being of historically underinvested communities by leading initiatives that build capacity through human-centered design, multi-sector partnerships and care delivery transformation.I have a longstanding commitment to serving and addressing upstream factors that adversely affect the health and equity of communities. I’ve served in a variety of capacities — program manager, facilitator, consultant, and trainer where I supported federally qualified health centers (FQHC) across the country. For more than 6 years I worked at a FQHC in Northern California where I oversaw the innovation portfolio. There, I used design and systems thinking methodologies along with process improvement to help problem solve and respond to the ever-evolving needs of the community. Some of my most notable work includes implementing a transportation solution that gave patients rides to and from their appointments, responding to homelessness using geo-mapping, convening and co-creating solutions with local organizations and public health agencies to address the local opioid epidemic, and addressing patients’ social needs with an online social service referral platform.Since joining the staff at the Center for Care Innovations (CCI) I’ve developed and managed learning collaboratives, provided training, consultation support, and technical assistance to safety net clinics, community based organizations and managed care plans with the goal of accelerating innovations and access to equitable care.
