Case manager February 2023 – July 21, 2023 • Provide substance use disorder counseling and case management services to clients• Maintain an individual client caseload of 10-15 clients• Create individualized treatment plans alongside clients• Complete all ASAM assessments, screenings, and discharge/transfer documentation• Support, facilitate, and participate in therapeutic treatment interventions for clients as needed• Collaborate with the clinical team to promote, facilitate, and achieve desired treatment outcomes for clients• Maintain accurate and timely documentation in compliance with all JTC, WAC, and company guidelines• Maintain ethical and professional boundaries in the counseling relationship• Develop individualized discharge and aftercare planning with clients• Respond appropriately to clients experiencing a crisis• Offer support and debrief client’s families on a weekly basis• Other duties as assigned
