The current role is working remotely to review and chart preparation for Medicare patients in a variety of clinical settings. The main goal was to review the patient problem list, with particular focus on the HCC codes ( Hierarchical Condition Category ). The goal is to maximize HCC reimbursement and capture HCC's in the EHR. Along with this review, the problem list was updated to reflect the complexity of patients. This work was performed on Medicare patients prior to their visits with the PCP.

When the Covid Pandemic affected my role as an Ambulatory Wellness Nurse, the company transitioned my team to work with patients during the pandemic. I was responsible for fielding phone calls related to testing, results and generalized inquiries. During this time, I was also responsible for a segment of the immunocompromised patients, where I followed them for re-testing and symptom management for up to 3 months. This position ended when permanent staff took over these roles.

This role consisted of seeing Medicare patients in the clinic where they visit their PCP. I completed the annual wellness visit for this population. The visit included reviewing their medications, problem list, allergies and immunizations. The visit also included basic vital signs, along with education on health care needs. The discussion would include Advanced directive, health care needs, nutrition and resources available to the patient. This position only ended due to the Covid Pandemic and the need to support the organization through this time.
