Deliver primary and palliative care services to over 150 medically fragile homebound patients in their homes as well as in Assisted Living facilities.

Dedicated to providing a personalized systems approach to the treatment of complex chronic disease using diet, lifestyle, nutrient, phytonutrient, mind/body, and pharmaceutical modalities. Provides Functional Medicine consultations to adults living with chronic disease seeking optimal wellness. Supports clients in integrating dietary and lifestyle adjustments to bring balance back to the body, and restore energy, hormone balance, digestive function, and overall wellness. Also provides care navigation services to patients and family members struggling with managing the complex medical care system.

Managed complex care needs of 70+ nursing home residents through frequent routine and urgent care visits. Developed and maintained strong relationships with residents, families, physicians, nursing home staff through frequent communication and coordination of care. · Collaborated with physicians to develop realistic plan of care based on resident’s goals; emphasis on contingency planning for high risk members to prevent unnecessary hospitalization.· Performed Serious Illness Conversations and reviewed Advance Care Plans continuously as patients moved through trajectories of chronic disease to adjust plan of care accordingly, avoid unnecessary treatments and improve well-being.· Adhered to model of care documentation requirements to include collection of quality measures and ICD-10 coding of all pertinent risk codes assuring accurate portrayal of member complexity. · Consistently met/exceeded benchmarks for key utilization metrics resulting in decreased unnecessary hospitalization, improved health outcomes and patient satisfaction. · Served on National Float Team – Western Region (territory CO/AZ/UT) starting 6/2019 to provide PTO and Leave of Absence coverage for Complex Care Management team.
