Director Of Community ResourcesBergen'S Promise Apr 2015 - PresentHackensackBergen's Promise is the designated Care Management Organization for Bergen County. We provide integrated Wraparound care coordination for youth up to age 21 living with with behavioral and mental health issues, substance use challenges, and/or developmental disabilities. Responsibilities include:- Maximize passion of integrated care to raise awareness and facilitate collaboration across departments and systems (education, healthcare, human services). Through formal partnerships and private/public grants, these collaborations aim to improve service and resource gaps for youth in Bergen County. Some of these include: *Coordination of Community Pediatrics and Child Advocacy Rotation with local hospitals *Partnership with federally qualified health centers and pediatric practices for the benefit of shared youth. *Collaboration projects with local hospitals to facilitate integrated care and seamless transitions of shared youth. *Connecting With Our Communities Youth Service Project for schools, scouts, faith based organizations to raise awareness of mental health and social determinant factors impacting youth and the local community resources available to support the youth dealing with these factors. *Through the BP Ambassador Outreach program and BergenResourceNet.org, people are connected to all the valuable programs, services, and activities available throughout the county. -As Co-chair of the Talent Engagement and Development Initiatives, she and her team support the organization’s recruitment and retention strategy, developing new creative recruiting initiatives to maximize staffing levels for all departments. They also support the planning and evaluation of activities designed to maintain and improve team member engagement and retention. Some of these include: *Establish partnership with universities to infuse Wraparound into the curriculum and establish social/human service career development and recruitment pipelines. -
