Within my role I used clinical reasoning, functional and cognitive assessments to address occupational performance and skills deficits resulting from stroke, to enable function in self-care productivity and leisure. I effectively communicated with patients with communication disorders (aphasia, cognitive communication disorder & dysarthria) and members of the MDT to identify goals to guide individualized treatment plans.I delivered individualized treatments plans focusing on optimizing independence in self- care, productivity and leisure. As well as delivering upper limb rehabilitation including ranging, tonal management, soft rolling, splinting and casting. I developed and co-ordinated comprehensive discharge plans alongside patients, families and the inter-disciplinary team.
Occupational Therapist- Complex Neurological Rehabilitation
Within the Complex Neurological Rehabilitation Team I used a range of specialist assessments and treatments to diagnose and treat physical, cognitive, behavioural, perceptual and psychological deficits resulting from neurological disease or trauma. This included the administration of standardised & functional assessments, postural and tonal management, cognitive rehabilitation and training and practice of daily living tasks. Using everyday activities to regain lost skills. I played a key role in providing safe and timely discharges from hospital, working closely with multidisciplinary teams, carers and community agencies to identify and arrange care needs, resolve environmental and housing issues and make appropriate referrals for on-going rehabilitation.
Occupational Therapist- Acute Surgical Team
As part of the Acute Surgical Team at St. James's University Hospital I provided in-patient specialist Occupational Therapy service across the following Clinical Service Units: Digestive Diseases, Heptorenal, Liver & Kidney. I delivered Occupational Therapy assessments and treatments focusing on individual patient’s functional abilities (including personal care and domestic skills) this also included provision of equipment, home environmental assessment and risk assessments.

The focus of this placement was to bring an Occupational Therapy focus to the Age UK charity. It aimed to bring an awareness to the Occupational Therapy profession and identify emergent roles outside of the NHS. This role was in line with current health and social policies within the UK such as 'The 5 Year Forward View', 'The Care Act' and 'Nice Guidelines'. The interventions I carried out on this placement included; lifestyle management, reminisense and life story work. The Occupational Therapy assessments administered included the Canadian Occupational Performance Measure.

The interventions I carried out included; functional assessments, standardised assessments such as The Model of Human Occupation Screening Tool & the Occupational Self Assessment. I also administered interest checklists, the Pool actively level and conducted cognitive assessments such as the ACEIII.Furthermore, I conducted therapeutic interventions such as behaviour experiments, grading/pacing and adaptations through equipment provision.This role also included care planning, assessment of risk and signposting to additional or alternative services where appropriate.
