Thiel Fellowship & Blueprint Health Winter 2015.Co-founded GlucoIQ, a population health platform, that allows ACO's, hospitals and physician groups to monitor chronic conditions such as diabetes and hypertension under Medicare's Chronic Care Management (CCM) program. We connect with glucose meters, fitness trackers and other medical devices to wirelessly stream data into the clinical workflow and generate both automated and human-assisted forms of intervention.
