Roll out the Graham SLAM (Support, Lead, Achieve, Model) Program, typically reserved for youth transitioning out of foster care, to a community based setting to effectively and consistently service West Harlem youth ages 15-23 at risk of system involvement. • Recruit and establish strong relationships with over 80 adolescents in the community to work one on one in creating individual goals and action plans toward graduation through post-secondary success. • Maintain hands-on mentorship approach with each youth; accompanying them across the city to school visits, court dates, medical/psych evaluations, social service appointments, program interviews, etc. • Work collaboratively across systems with various service providers and stakeholders to identify and secure long term, wrap around academic, emotional, and vocational support for each youth in a coordinated fashion • Spearhead the start-up of Graham’s Bengals program into the agency’s foster care and prevention divisions; designed curriculum, recruited youth, facilitated trainings to double number of participants • Facilitate a recurring series of therapeutic peer groups focused on attaining Vocational Readiness, College Access/Retention as well as Peer Leadership & Community Service.
