Advocating for vulnerable populations.Assist peers transitioning from mental health hospitals into independent living or other. Create service plan/goals, crisis plan and emergency plan. Assist with creating a transitional items list and other home goods. Collaborate with community agencies; Aging and long term care, Home and community services, Yakima Housing Authority, Payee; Spokane Housing Authority, local mental health and substance abuse facilities and other behavioral health services. Assist in applying for home subsidy wherever eligible. Locate housing and advocate for peer when obtaining new lease. Request funds approval following both internal and external guidelines with other case managers. Assist landlord with housing inspection process; Upon approval secure housing. Prepare discharge plan and establish new PCP, Mental health provider, pharmacy, transportation and other recommended referral’s. Assist client in orientating to new home setting and follow up with after care providing in person visits, support when needed and obtain permanent subsidy. Report back to care team.

(Temporary) Work from home and out in community within Yakima county. I offer new members medical and social resources. Make calls daily, monitor client interactions and work on plan of care such as; Finding a new provider, status of medical referrals, schedule appointments, connect members with transportation, food banks, housing, transport clients to medical appointments, assist member in understanding treatment plan and health care benefits and also connect with other resources and or services.

(Temporary Contract position; (Remote work)) Contact trace, Assigned cases to obtain data for all exposed and infected cases, investigate each case within procedure. Later promoted to Senior Case Investigator; Train and over see new staff, assign new cases to team, working with various computer programs and assisting community’s throughout Washington State. Transferred departments due to lay off; as a Clinical assistant, help organize and test patients for Covid-19, we later transitioned into vaccines where I also assisted in vaccination registration for farm workers throughout Washington State counties. Contracted with both Washington State department of health and medical teams international.

(Temporary 90+ day contract) Monitor unaccompanied children. Assist in daily activities. Support all staff wherever possible. Guide unaccompanied children; follow daily schedule. Educate and encourage children throughout the day in a positive manner. Refugee Children mission.

Working with a vulnerable population who is in need of a lifestyle change. All clients are in recovery suffering from addiction. Transporting clients on a daily basis, assist with coping, intakes and discharges. Medication management are part of the program needs.

Long term care, assist residents with activities of daily living. I assisted 8 residents in the household. Prepared meals, assist with medications.

Home care- Assist a vulnerable population for those who need the extra help with daily activities. Monitor client, assist with medication, appointments, food prep, hygiene and grocery shopping.

Competency Restoration program with clients who did not meet competency guidelines in court to stand trial. My duty as a case manager was to educate the client in court room procedures in order to stand trial. With this type of work I assisted clients with everyday needs. For example, Coping strategies, daily activities such as educational groups and learning coping techniques. As a case manager and working for a new program, I worked closely with clients, Psychiatrist, team leads, program directors and therapist to meet all treatment goals. Having the ability to adapt quickly and deescalate clients when necessary with something we call "Pert training" (psychiatric emergency response certified). Precautionary measures were taken by a step by step technique, Some clients did not have the ability to cooperate and sometimes were very physical. I was trained to guide the client to safety by this "pert training". Also, utilized psyche consult program to document/report all behaviors which pertain to client barriers and treatment goals.
