Provide oversight and monitors all components of Medicaid waivers and services of individuals on a caseload of 35-40 individuals. Assists individuals to navigate, apply, and maintain community and State benefit resources. Works with inter-disciplinary teams to assess needs and to develop and implement plans to maintain health and safety. Evaluates and performs multiple State and County assessments. Provides oversight, monitors, and reviews service delivery of agencies providing services to each individual. Travels to meet with agency staff, family, advocates, and other agency members to monitor and review services being provided to individuals with developmental disabilities. Establish budget that is fiscally responsible for County funds for each individual while providing services necessary for health, wellness, and safety in the community and at home.

Provide case management and housing stabilization services to support an average of 24 individuals transitioning from chronic homelessness into supportive housing. Develop treatment plans based on clients’ needs, objectives, and personal goals for those who are homeless, mentally ill, chemically dependent or abusing substances, or may be involved in the criminal justice system. Demonstrate skills including clinical assessment and treatment. Work closely with a small treatment team in order to provide comprehensive support based on clients’ needs. Meet with each client twice weekly face-to-face to ensure treatment reflects current needs.

Offer community-based support and psychosocial rehabilitation to an average of 15 clients suffering from severe and persistent mental illnesses, providing support based on each client's needs. Develop individual recovery plans, define goals and objectives, and create intervention plans based on comprehensive review and analysis of annual mental health evaluations. Serve as an advocate for each client whether attending physician appointments, accompanying clients during activities of daily living, or managing their medication; provide the support and access the resources critical to promoting daily independence.

Engaged in a customer-centered operation, coordinating loss mitigation and default management cases in the residential mortgage arena. Connected with loan holders to qualify for mortgage modifications, working diligently to mitigate risks associated with foreclosure and bankruptcy actions. Managed workflow with efficiency and effectiveness, meeting all production targets. Cultivated professional relationships with clients during challenging situations, educating loan holders on options to protect their best interests-and property assets-while mitigating losses and reducing overall exposure for the bank.
