• Perform psychiatric evaluations, treatment, medication management, counseling, and coordination of care for high-risk children and adolescents ranging from 5 to 17 years old. Population consists of youth in Department of Child Safety or non-biological guardian custody suffering from trauma, behavioral disturbances, learning disabilities, autism spectrum disorder, and addictions.• Provide medication monitoring and facilitate counseling interventions, psycho-education, illness self-management, and family/stakeholder involvement.• Coordinate crisis planning and intervention in-office and after-hours as needed.

• Complete inpatient psychiatric evaluations and consultations of individuals experiencing crisis, opioid use disorder, homelessness, and mental illness at Little Colorado Medical Center.• Provide direct psychiatric care utilizing a whole-health, integrated care approach to treating children, adolescents, and adults designated as General Mental Health (GMH) or Seriously Mentally Ill (SMI).• Provide psychiatric evaluations and medication management for individuals with co- morbid, medically complex conditions, dual diagnosis, substance use, and homelessness.• Manage the utilization of services with a focus on managing the most intensive and highest levels of care including minimizing preventable inpatient and emergency department admissions related to behavioral and physical health.

Family Psychiatric Nurse Practitioner• Recruited to support the addition of Children’s Services within West Phoenix. After achieving 40% increase in new clients over prior year, assisted the child and adolescent expansion into the East Valley with intra-agency support via Telemed/Telepsychiatry. • Provide direct psychiatric care utilizing a whole-health, integrated care approach to treating children, adolescents, and adults designated as General Mental Health (GMH) or Seriously Mentally Ill (SMI).• Provide psychiatric evaluations and medication management for individuals with co- morbid, medically complex conditions, dual diagnosis, substance use, and homelessness.• Manage the utilization of services with a focus on managing the most intensive and highest levels of care including minimizing preventable inpatient and emergency department admissions related to behavioral and physical health.• Proficient in NextGen and ClaimTrak electronic medical records (eMR).

• Provide direct psychiatric care within the community mental health setting to individuals with moderate to Severe Mental Illness ranging in age from 4 to 80 years old. Provide psychiatric evaluations and medication management for individuals with co-morbid, medically complex conditions (HEP C, HIV), dual diagnosis, substance use, and homelessness.• Maintain a daily caseload of 3-6 psychiatric evaluations and up to 20 medication management appointments, which includes treatment planning and coordination with in-clinic, medical, and community resources. • Proficient in Avatar electronic medical record (eMR) and OrderConnect e-prescribing.
