I am a results-oriented, process driven Licensed Practical Nurse with over 30 years of experience. I have been working as an MDS Coordinator for the past five years and have been at this skilled nursing facility for the past 15 years in a variety of roles, including a charge nurse and the Assistant Director of Nursing. As the MDS Coordinator, I was responsible for scheduling and completing as many as 25 MDS assessments monthly in an accurate and timely manner per state and federal regulations, creating and updating accurate and detailed person-centered care plans with goals and interventions appropriate to meet the residents' needs and facilitating communication with residents, families, staff and physicians regarding the plan of care.
