Oversight of a caseload of 25-40 men with historical substance use experiencing chronic homelessness. Diligent case note writing for bi-monthly meetings, consistent follow-up on Incident Reports. Collaborated with several departments concerning clients such as work readiness supervisors, security staff of The Crossing, mentors, life skills teachers, and more. Assisted directly with a licensed counselor, working on a Care Team, for each participant to guide progress in the program and assist with behavioral needs. Upheld the standard of a sober-living environment for men that lived in the facility. Updated case plans each month that documented the goals of the participant, and a Self-Sufficiency Matrixes every 3 months to track increase or decrease in participant’s self-sufficiency scoring, and collect data for how programs and services were progressing. Partnered and maintained professional rapport with Mentors, outside peer to peer recovery services, and other agencies that supported clients in the program. Assisted participants in retrieving legal documents back, such as Identification Cards, Birth Certificates, and Social Security Cards, sometimes for participants that did not have documentation for several years. Assisted with legal needs such as court appearances, probation progress, parole progress, child support, DUI cases, and much more. Tracked progress and multiple phases of program requirements that participants were following, ensuring they would meet goals set for each phase of the program and complete them time. Employment and job search assistance, resume building assistance, performed many O*NET career assessments directly with participants who were interested in beginning a new career. Budget and savings assistance once participants gained full-time employment. Housing assistance for participants as they neared the end of their time in the program.
