As a Service Co-ordinator I am responsible for ensuring services are delivered efficiently, safely and effectively through client-negotiated care plans. I oversee their implementation, monitor and review services provided by Care Workers to new and existing Home Care clients and ensure that the most appropriate Care Worker delivers the service.I provide individualised service, care planning and coordination to support the delivery of in-home support services to the frail aged, people with disabilities and their carers, and form networks, where possible, to link with other community providers on behalf of the client and their carers.I am responsible for clearly identifying, properly assessing, effectively controlling and regularly reviewing safety hazards in the workplace.As a Service Co-ordinator I am required to effectively manage worker's compensation claims to achieve timely and appropriate rehabilitation of injured workers.Choosing the most appropriate Care Workers to deliver services to new and existing clients; I make recommendations to the Branch Manager to recruit or subcontract staff where there are skill or staff shortages; I make decisions regarding staff attendance at training; including recommendations to management regarding the day-to- day management of service delivery under the provision of current OH&S legislation. I have the ability to interpret feedback from staff to ascertain whether a client's care needs have changed and a reassessment is required. There have been times where I have been required to facilitate and participate in both internal (e.g. Care Worker Teams) and external committees, case conferences and meetings.I would use SAP Employee Self Service (ESS) on a daily basis as it allows me to manage my teams leave, calculate leave projections, and review care workers and services co attendance at work, payments and personal information online.I would use Workforce Central on a daily basis to arrange clients services and schedule
