• Responsible for providing in-home services to children/adolescents and their families to increase stabilization for families involved with the Department of Child Protection and Permanency (DCP&P) • Provide supportive family and individual contacts, transportation, case management, assessments, intervention planning, client/family education and after care planning. • Document services in clinical record, engage in inter-organizational care coordination, and document service outcomes. • Formulate relevant goals with behaviorally measureable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement.•Plans for, communicates, and document aftercare plans for clients completing program and/or accessing other services in the continuum.• On-call responsibilities for crisis intervention
