• Project implementation by strictly adhere to the study protocol at all times • Identify and retrieve hospital records in collaboration with the clinical team and hospital records officers and return these to a location as directed by the hospital team once data entry is complete • Identify and retrieve hospital records for study participants in collaboration with the study clinician, hospital records officers, pediatrician, nursing officer in-charge of the paediatric unit and return these to a location as directed by the hospital team once data entry is complete • Maintain organization and tracking of all study-related documents and data • To abstract relevant medical / nursing information from medical records into a computer database as directed during training • To track essential laboratory and discharge data linked to medical records and enter into the database as directed during training • To track essential post-discharge data from study participants linked to medical records and enter into the database as directed during training • To attend monthly pediatric mortality meetings and report back on the hospital team’s feedback on data quality • To submit data to the central data managers as directed during training and as requested • To check records where data entry errors are suspected and make corrections with the support of the study clinician and the central data management team. • To report to the hospital team on a regular basis the progress being made and highlight any difficulties that arise • To ensure in partnership with the hospital that the database resulting from the data entry is kept in a secure location on the hospital’s computer network.• To ensure the security of the computer, study documents, supplies and equipment at all times and by adopting good working practices protect the computer and other equipment from loss or damage • To maintain confidentiality at all times when dealing with medical records
