Perform complete MTM's (Medication Therapy Management) on specific patients with specific insurance groups. My purpose and goal is to optimize drug therapy and improve therapeutic outcomes for the patients. Work closely with Physicians, Nurses and Case Managers in discharging patients with the appropriate medications. Obtain prior authorizations for medicines prescribed outside the preferred formulary for UHC and AMG patients if unable to work with doctors on a different therapy.I performing patient assessment /a comprehensive medication review, help create a medication treatment plan, monitor efficacy and safety of medication therapy, enhancing medication adherence through patient education, and documenting and communicating MTM services to prescribers in order to maintain comprehensive patient care. Furthermore, I obtain the actual prescriptions from the physician and send them off to get processed and have them delivered back before the patient is actually discharged or set up arrangements to have them delivered to the patient. Upon patient leaving I council them on their medicines (DDI, SE, adherence and the proper way to use).Then complete proper reports so that the patient is followed up by the appropriate follow up calls.

Spoke to physicians with regards to providing alternative medications to ones not covered due to the closed formulary. My responsibilities other than the standard services of a pharmacist and manager were interviewing pharmacists for the company. I created and updated a floating RPh schedule for the Las Vegas area. Helped open new stores for the company. Assisted regional pharmacy manager Ellie Gilbert with all types of issues and problems in the Las Vegas area when was out of the area.
