Designed transition readiness assessment tools for adolescent patients, parents, and providers to gather 360-degree feedback on measure of readiness to transition from Lurie Children's to an adult provider.Concluded with presentation of pilot results and recommendations to improve program delivery and patient outcomes as well as a toolkit for assessment implementation.

Responsible for all recruitment, training, placement and cultivation for approximately 300 volunteers as well as Corporate and Community Partner relations.
