Part of Key Leadership of the ACO in building Care Coordination efforts that were highly successful in reducing admissions by 100/1000 with limited access to real time admission data at all hospitals in the catchment area. ACCENC was able to reduce readmissions to the Local hospitals from 22% to 14%. First year losses were in the range of 5.5 million dollars and chief architect of redesign and redeployment of Care Coordination and practice engagement plans that turned around the losses and eliminated them. The ACO was a partnership between The Beacon Company and Collaborative Heath Systems who declined to continue participation and the ACO was closed by mutual agreement. GPRO reporting was accomplished for the initial and subsequent years using local team personnel and with CHS support. Patient satisfaction scores were among the best in the Nation.
