HAAD Registered Pharmacist 1 since 2015nsures that verified medication orders are properly filled• Delivers duly checked medications to the respective patient care areas• Knowledgeable with aseptic preparations techniques, prepares all sterile admixtures and handles hazardous medications preparations, and prioritizes medication production appropriately• Maintains required records and documentation• Responsible for all Pyxis machines making sure that medstations have sufficient inventory and are working properly• Responsible with proper use and maintenance of equipment such as repackaging equipment, Pyxis medstations, compounding equipment, etc.• Daily monitoring of medication outdates• Monitors the appropriate stock levels of drugs and supplies

• Assigned to critical care areas (Emergency Department, Medical Intensive Care Unit, Neuro-Surgical Sciences Intensive Care Unit), and units where average number of drugs per patient is 7 to 10• Attends to all drug related functions in the unit: medication order review, drug appropriateness review, reviews and checks for accuracy and completeness of patient drug profile and medication administration, drug utilization review, medication reconciliation, patient medication counseling• Orients and trains new clinical pharmacists, trainees, interns and post-graduates• Gives assistance during cardio-pulmonary resuscitation: takes note of the time CPR started/ended, prepares the medication as prescribed by the doctor, makes a list of medications given as well as time administered• Provides pharmacy intervention• Provides drug information to doctors, nurses, patients and relatives• Checks the completeness and expiration dates of drugs in the floor stock of assigned unit• Ensures the compliance of standard storage requirements of medications• Coordinates with inpatient pharmacy and drug-related concerns of consultants/doctors/nurses• Prepares skin test solutions of intravenous antibiotics, iron complex solutions, vitamin B complex solutions, analgesic preparations and other drugs at the emergency department

• Responds to medication-related queries from within the hospital, other hospitals and the general public• Ensures the proper dispensing especially of unit-dose drug dispensing and timely distribution of drugs for inpatient or outpatients• Compounds extemporaneous preparations• Ensures patient safety by providing drug therapy monitoring and pharmacy intervention• Monitors every stage of medication therapy to improve all aspects of delivery and reporting patient side-effects• Ensures medical products are stored appropriately and securely to ensure potency and stability
