• Provides services to individuals with physical and intellectual disabilities and to those with persistent mental illness• Researching services and resources provided by community organizations in order to assist individuals with gaining access and increasing accessibility in the community • Facilitates the following clinics for 150+ individuals with mental/behavioral health issues and/or those with intellectual disabilities agency-wide: psychiatric, medical, dental, physical/occupational therapy, optical, and podiatric• Develops collaborative individual program plans as a part of a clinical treatment team, with client input, during bi-annual individual program planning meetings, that ensure provision of all covered services identified in the program plan, referrals to community resources as appropriate; and continuity of care between inpatient and outpatient settings, services and supports, as applicable.• Insures maintenance of all client social service benefits• Completes an array of case management assessments of clients.• Completes all appropriate paperwork for client charts and maintains comprehensive clinical record.• Serves in an on-call capacity as scheduled to facilitate crisis management and coordinate emergency medical and/or psychiatric hospitalizations as needed• Plans, organizes and implements recreational activities designed to increase clients' wellness and community integration.• Facilitates and monitors any ancillary services, benefits or entitlement that may augment client progress and treatment.• Documents and bills for all services rendered to program participants in the required time frame and in accordance with all Medicaid Rehabilitation Option (MRO) guidelines• Performs other duties as assigned.
