Engage & coordinate MAC service providers & supports - Refer to allied health - Support clients to remain living safely & independently in their own home - Link clients with mainstream, formal & informal supports - Provide budget monitoring & manage Home Care Package funding - Maintain accurate case notes - Maintain up to date knowledge of MAC & NDIA guidelines.
