Investigates adult abuse / neglect allegations to include visiting homes and working with family members, community professionals, law enforcement and court system. Conduct assessments of the vulnerable adults situation, mental and functional abilities and safety risks. Provide emergency interventions; participate in the legal process including case preparation and testimony. Evaluate services and care needs, develop case plans, arranges and coordinates services to ensure safety and reduce risk.

Supervise and evaluate the work of Service Coordinators by effectively recommending personnel actions related to selection, work schedules and assignments, performance, leave requests, grievances, and disciplinary procedures. Provide technical assistance and training to Service Coordinators and Assistants in completing ICAP’s, eligibility determination processes, Intake, Individual Family Support Plans and Individual Program Plans. Assist in developing, interpreting, and implementing policies, procedures, and regulations related to the provision of service coordination. Monitor implementation of policies and procedures and regulations. Provide technical assistance to the Service Coordinator in gathering evidence and preparing information for the client due process mechanism for eligibility determination and service provision appeals; communicates appeal information to the Service Coordination Local Manager. Member of the Transition Committee (School to Community)Supervise and participate in the process of specialized placement of individuals with dual diagnosed needs.Supervise and participate in the process of community placement of BSDC residents.Member of the Transition Committee (BSDC).Manage the efficient use of staff and resources to include: organization of work, size of caseload, acquisition of equipment or other needed resources. Compile area registry of clients needing services as well as clients receiving services to ensure the availability of useful information for policy makers, state officials, and others to support the statewide planning, management, and budgeting process.

Coordinate the development and implementation of the Individual Program Plan / Individual Family Support Plan in conjunction with the individual, the family and service providers.Conduct ICAP’sMonitors and evaluates the effectiveness of the direct services on an ongoing basis through observation and review of related written documentation.Investigate any health and safety concerns and reports to appropriate agency.Serve as an advocate for individuals or caseload to ensure that personal preferences and needs are met.Secure and/or arrange services for individuals as determined by the IPP/IFSP. Conducts and documents at least one monthly contact with or on behalf of each individual’s service coordination file.Serve as the individual’s liaison by receiving and communicating pertinent information to providers of services, parents/legal guardians, and/or family as appropriate.Explain the due process mechanism to individuals and families as needed.Work with Individual Education Plan team as needed to ensure service continuity across settings and in developing transition plans. Assist individuals in managing personal finances as determined by the team.
