Als GHV’er en ambulant begeleider ben ik verantwoordelijk voor het ondersteunen én motiveren van mijn cliënten in hun dagelijkse verrichtingen en hulpvragen om zo hun zelfredzaamheid en eigenwaarde te vergroten. Daarnaast informeer en stimuleer ik het (sociale) netwerk van en mét de cliënten en onderhoud ik het contact met partijen als bewindvoerders, curatoren etc.Het betreft hier cliënten met zowel chronische somatische klachten alsmede verschillende psychische en psychogeriatrische aandoeningen en ontwikkelingsstoornissen in verschillende levensfases.

In deze functie ben ik het eerste aanspreekpunt voor klanten, bezoekers en collega’s. Ik ben samen met mijn collega’s verantwoordelijk voor het efficiënt beheren van verschillende informatiestromen, maar ook voor het verwerken van de uiteenlopende zorgaanvragen via Point en Zivver. Daarnaast voer ik verscheidene archiverende en administratieve taken uit binnen ONS. Ik behandel inkomende MIC-formulieren en voer enquêtes uit met uit dienst tredende collega’s. Ik bereid vergaderingen en evenementen voor en beheer zowel de agenda alsmede de kantoorvoorraad.

Het is onze missie om ervoor te zorgen dat bewoners van onze kleinschalige woningen hun leven naar hun wensen en behoeftes kunnen voortzetten met behulp van individueel afgestemde, belevingsgerichte zorg. As a trainer all throughout the country, I raise awareness among students at primary and secondary schools about risk behaviour and peer pressure through programs such as "Split The Risk" and "Fight Your Inner Monkey". To achieve this, I write and implement business plans according to the Business Model Canvas standard, focusing on expanding the catering on the farm by cooperating with local businesses and developing educational nature and environmental activities, all focusing on the clients' active participation. De Woesteland Academie wilt jongeren bewust maken van het belang van duurzaam gedrag voor onze aarde en hen de Nederlandse natuur op waarde leren schatten. Om dit te bereiken wilt de WLA dat zoveel mogelijk mensen die in contact staan met jongeren weten hoe ze (andere) jongeren op een positieve manier kunnen boeien, interesseren en betrekken bij natuur en duurzaamheid door middel van non-formele educatie. Via workshops leren de deelnemers om zelf natuur- en duurzaamheidsactiviteiten vorm te geven waarbij jongeren worden geïnteresseerd, geboeid en geprikkeld. Ik ben verantwoordelijk voor het ontwikkelen en trainen van twee workshops;- Storytelling; de deelnemers leren om verhalen die aansluiten op de belevingswereld van jongeren als krachtig communicatiemiddel in te zetten om hen te betrekken en te inspireren duurzaam te leven. - Duurzaam Ondernemerschap voor Jongeren; de deelnemers krijgen tijdens deze workshop concrete handvatten aangereikt om hun idee voor een (eenmalig) duurzame initiatief of bedrijf direct tot een plan uit te werken en deze te presenteren.

Daarnaast geef ik ook rondleidingen door het stadspark waarbij ik o.a. onderwerpen als diergedrag, de ontstaansgeschiedenis, de architectuur en de kunst en de rol van de tuin in de Tweede Wereldoorlog behandel.

In een kleinschalige woonvorm ondersteun ik als persoonlijk begeleider 12 verstandelijk beperkte mensen, variërend in een licht tot ernstig beperkingsniveau, vaak in combinatie met een pervasieve ontwikkelingsstoornis en/of een psychische stoornis. Dit gebeurt via het principe van versterkend begeleiden (Emancipatorische Methodisch Kader, Karel de Corte, 2014), waarbij de nadruk ligt op het zoveel mogelijk vergroten van de autonomie van de cliënt.Mijn dagelijkse werkzaamheden bestaan uit het ondersteunen van de cliënten in aspecten als financiën, gezondheid, dagbesteding, huishouding, communicatie/sociaal, sport etc. Leidend hiervoor is wederom dat de cliënt zoveel mogelijk de regie voert in het eigen leven.Daarnaast ben ik verantwoordelijk voor het opzetten van individuele ondersteuningsplannen (via het principe van methodisch werken in het huidige Elektronisch Cliënten Dossier, ECD) in samenspraak met de cliënt, wettelijk vertegenwoordigers, collega's en andere disciplines. Mijn individuele teamtaken omvatten; - het beperken en verantwoorden van de inzet van middelen en maatregelen op de woning (Wet BOPZ). In mijn functie als persoonlijk begeleider voor dementerende ouderen richt ik mij vooral op het in kaart brengen van de aanwezige cognitieve en motorische eigenschappen en vaardigheden en de sociale situatie bij deze doelgroep. Vanuit deze voorkennis definieer ik zorgvragen van waaruit ik de bestaande vaardigheden, eigenschappen en het sociale systeem probeer in stand te houden of, indien mogelijk, uit te bouwen. Daarnaast bedenk en implementeer ik (veelal individuele) activiteiten op het gebied van lichaamsbeweging, muziek, reminiscentie, contact met dieren en snoezelen, waarbij de beleving van de cliënt en het stimuleren van de zintuigen centraal staat. Door cliënten te ondersteunen, motiveren en stimuleren tijdens de ADL en voorgenoemde activiteiten probeer ik de zelfstandigheid, zelfredzaamheid en eigenwaarde van mijn cliënten te vergroten. Autonomie van de cliënt is hierbij.leidend. Verder onderhoud ik contacten met verschillende disciplines en wettelijke vertegenwoordigers t.b.v. bovenstaande en werk ik vrijwilligers in op activiteiten.
