Providing short term therapy in a primary care medical clinic through an integrated model

• Manage a team of 6 clinicians to act as a resource and advocate for both the agency and the clinicians• Provide supervision on a monthly basis to ensure clinicians’ needs are being met, to allow them to process through any challenges and/or successes, and to ensure agency protocols are being fulfilled• Review documentation to confirm all notes meet insurance requirements and SMART measures• Meet weekly with a caseload of 15-20 individual clients and utilize evidence based practices such as CBT and DBT to help foster improvement, growth and meet clients’ self-defined goals• Complete documentation in a timely manner to ensure clients’ continued access to therapeutic services• Collaborate with clinical peers to receive support and guidance in a weekly supervision meeting

• Collaborate in a multi-disciplinary team in order to provide excellent and effective care for clients• Carry a caseload of 8-10 adults experiencing the highest level of severe and persistent mental illness• Connect clients to basic needs such as housing, public benefits, medical care, psychological needs, financial resources, legal needs, employment services and community resources• Assist clients in developing their own goals through treatment plans and needs assessment • Provide strengths-based case management services to develop positive relationships with clients • Empower clients to work towards their own independence and stability • Document client progress in a timely manner• Advocate for and support clients in order to promote success

• Carry caseload of up to 7 individual clients struggling with substance use and mental health diagnoses• Co-facilitate IOP group therapy for young adults utilizing CBT, DBT and MI focused interventions• Facilitate IOP group therapy for adolescents utilizing CBT, DBT and MI focused interventions• Co-facilitate a combined OP group therapy for young adults and adolescents• Document progress of clients through the use of DAP notes• Conduct intakes for new clients• Coordinate client care through assessing clients’ needs, treatment goals, and family systems • Provide referrals to outpatient community based therapists to prepare clients for discharge• Discharge planning

• Carry caseload of up to 7 individual clients struggling with substance use and mental health diagnoses• Co-facilitate daily young adult (19-30) and adolescent (13-18) IOP group therapy utilizing CBT, DBT and MI focused interventions• Co-facilitate a weekly combined young adult and adolescent OP group • Document client progress through the use of data, assessment and plan (DAP) notes• Conduct intakes for new clients comprising of explaining client rights, filling out releases of information (ROIs), discussing treatment expectations, explaining grievance policies and general paperwork • Coordinate client care through assessing clients’ needs, treatment goals, and family systems • Facilitate discharge planning through providing referrals to outpatient community based therapists
