Established the Social Work Department in a brand new hospital. Policies and procedures developed. Built relationships with teams throughout the Hospital, and with physicians to develop the best discharge outcome for patients to reduce re-admissions.I resigned after giving birth to my second set of twins.

Worked throughout the hospital with numerous multidisciplinary teams to provide case management and discharge planning. This experience includes the Emergency Department, NICU, Ortho, Trauma, Heart and Lung Transplant, Bone Marrow Transplant, and ICU. Working directly with the case managers in the hospital and the insurance case managers to develop a safe discharge plan. Providing education to the patient and their caregivers on their choices related to insurance guidelines. Making referrals to Rehab Facilities, Skilled Nursing Facilities, Nursing Homes and Hospices.

Responsible for coordinating dismissal plans of assigned hospital patients. This included but was not limited to facilitating Nursing home placements, Hospital transfers out of state, Hospice care and Home Health Care with patient, their family, their insurance company and the receiving health care provider, along with arranging transportation and required durable medical equipment.Completed psychosocial assessments on patient's going through the Liver Transplant Clinic. Working a a member of a multidisciplinary team evaluating a patient's ability to endure the transplant process both medically and psycho-socially.
