Interim Executive Director. Act as a conduit of information for the community and business owners. Create and curate a variety of content to grab attention and keep information up to date. Generate and edit content for weekly newsletter. Communicate professionally with KPAA businesses, community members and board members.

Assist in the daily operations and delivery of community based self- management programs. Responsible for data entry, generating data reports, ordering program supplies, communicating with patients, patient enrollment and delivering an evidence-based curriculum. Perform home-based services including assessing family/patient needs; developing intervention and referral plans with individuals and families as per pre-established protocols. Provide advocacy services requiring extensive knowledge of community resources. Perform a variety of administrative functions associated with the Community Health Team program operations to include cold calling, patient and participant recruitment and enrollment, scheduling, program promotion, program participant registrations, travel arrangements, data generation and reporting.
