Responsibilities: Perform medical record abstraction for collection of hospital Quality Measure data and other data as assigned. Essential Duties and Responsibilities include the following. Perfor abstraction of selected medical records within specified time frames for commercial clients. This includes any or all of the following: Apply data element definitions and abstraction instructions found in applicable manuals to accomplish timely and accurate review of selected medical records. Maintain up-to-date knowledge of the applicable Specifications Manuals as well as periodic updates and program guidelines affecting the Quality Measure abstraction and reporting. Identify incomplete medical records (e.g., ED nursing record is missing from the submitted copy) and submit requests for missing sections of the medical records to the Project Manager; complete abstractions when missing sections are made available. Identify deficiencies in the existing medical record documentation (e.g., documentation addressing a particular data element is either not captured in the finalized record or entries contain conflicting information) and submit suggestions to the Project Manager on ways to improve in documentation. Search relevant online question and answer databases, such as those made available by CMS and the Joint Commission or other registries, to resolve unclear documentation scenarios; utilize other abstraction resources such as appropriate listservs. Refer abstraction questions to Project Manager or Director as needed. Accurately enter abstraction results into applicable data entry tools designated by the organization. Complete review and data entry within assigned timelines. Work with Project Manager and Director to ensure all record abstraction deadlines are met.
