Creating and providing day supports to individuals with developmental disabilities with various diagnosis, service requirements and complex needs; constructing opportunities for individuals to access community in recreation, employment and volunteerism capacities. • Co-Supervisor of five day programs: Recreation and Leisure, Employment, Micro-Business, Outreach and Seniors.• Focus on forming and developing relationships with business owners in communities of Edmonton and St. Albert; forming meaningful employment and volunteerism opportunities for supported individuals.• Focus on continued community inclusion and social integration, development of natural supports. Forming community partnerships with emphasis on opportunity for personal care and therapeutic approaches to maintain high level of well-being.• Implementation of new strategies to keep participants physically active and challenged.• Development of Individualized Service Plans; focus on creating positive outcomes referencing health, complex behaviors and personal interest. Meeting Quality of Life Domains, participating in Supports Intensity Scale assessments.• Meeting deadlines, paperwork and documentation, nominal roles, incident reporting, risk assessments, person centered planning, goal development, restrictive and positive procedures, medication administration.• Supervising staff, maintaining communication with necessary contributors including staff teams, guardians, public offices, employers, health specialists. Managing team to ensure thorough communication and investment into ensuring quality of life is maintained for caseload of approximately fifty individuals. • Working within policy and procedures to create positive working environment for staff, individuals, families and guardians.• Creating Excellence Together committee member, updating individual files, agency policy and procedures to meet standard of excellence as deemed by provincial regulation. 100% achievement.
